Service Project Reports

Reminder:  Every student must do six hours of service each semester.  This service may notbe performed within your immediate family and you may notreceive any money for the service.  After the project(s) is (are) completed, you are required to write a report about the service.  The format for the report will be specified below.  The supervisor or person for whom the service was done is asked to sign for verification of service.  If this person is unavailable to sign the paper, a parent or guardian must sign and explain why the person is unable to sign. The service project is graded as a major project.

Requirements:

1.    

A total of 6 hours of service (either one project totaling six hours or several projects totaling six hours)

2.   

Cover sheet with the appropriate signatures (note: grade will be lowered if this sheet is not turned in)

3.   

A written report that is 2 pages in length.

The following questions are to be answered in the report.

What was the actual service project you performed?  Be specific and detailed.
